What information we collect
We may collect the following types of personal information:
Your name, date of birth, contact details (phone number, email address, and postal address), health information relevant to your chiropractic care, appointment history, payment information, and any other information you provide when contacting us or completing forms on this website.
We collect health information because it is necessary to provide you with safe and appropriate chiropractic care.
How we collect your information
We collect information directly from you when you complete an online booking or enquiry form, contact us by phone or email, complete a new patient health history form, or visit the practice in person.
We do not collect personal information from you without your knowledge.
How we use your information
We use your personal information to provide and manage your chiropractic care, communicate with you about appointments, respond to your enquiries, comply with our legal and professional obligations as a registered healthcare provider, and improve the services we offer.
We do not sell, rent, or trade your personal information to third parties.
Disclosure of your information
We may share your information with other healthcare providers involved in your care where you have consented or where it is necessary for your treatment, our practice management software provider operating under appropriate data security arrangements, and regulatory bodies such as AHPRA where we are required by law.
We will not disclose your health information to any other party without your consent, except where required or permitted by law.
Health records
As a chiropractic practice, we hold health records that are subject to additional protections under the Health Records Act 2001 (Vic) as well as the Privacy Act 1988 (Cth). Health information is stored securely and retained for a minimum of seven years from the date of your last consultation, or until you are 25 years of age if you were a minor at the time of treatment, whichever is longer.
Patients under 18
We welcome patients under the age of 18. Where a patient is under 18, a parent or legal guardian is required to provide consent for assessment and treatment. Personal and health information relating to patients under 18 is handled with the same care and confidentiality as adult records and is retained in accordance with the timeframes above.
If you are under 18 and wish to make an enquiry or booking independently, we encourage you to speak with a parent or guardian before your first appointment.
